Cityblock, Sunshine Health Partner to Deliver Care to Florida Medicaid Members in Central Florida

What You Should Know: 

– Cityblock, a value-based healthcare provider specializing in Medicaid, announced a new partnership with Sunshine Health, a Florida managed care plan. This collaboration aims to provide high-quality primary care and care coordination services to Medicaid beneficiaries with complex needs in Central Florida.

– The partnership expands Centene Corporation’s, Sunshine Health’s parent company, existing relationship with Cityblock. Cityblock has already established successful partnerships in other states with high Medicaid populations, including New York and Ohio. 

Focus on Accessibility and Whole-Person Care

The partnership, launched on May 1st, 2024, prioritizes accessibility for members. Cityblock’s unique care model offers 24/7 wrap-around services, allowing patients to connect with their multidisciplinary care team virtually, in-home, or at local clinics. This flexibility ensures patients receive the care they need, when they need it, regardless of location.

Serving Underserved Communities

The program initially targets members in eleven Central Florida counties: Volusia, Seminole, Orange, Osceola, Polk, Hernando, Pasco, Hillsborough, Pinellas, Manatee, and Sarasota. These counties often house Medicaid beneficiaries facing complex medical and social challenges. Cityblock’s model is specifically designed to address the needs of such populations, aiming to improve health outcomes and overall well-being.

Reducing Costs and Improving Outcomes

Cityblock’s data-driven approach emphasizes preventative care and coordinated support services. This approach has been shown to reduce unnecessary emergency department visits and hospital admissions, ultimately lowering healthcare costs. According to Cityblock’s 2024 Equity in Action Report, their care model has demonstrably improved patient engagement, ensured clinical continuity, and significantly reduced avoidable hospital admissions.
