– CoachCare, a provider of remote patient monitoring (RPM) and care management platforms has acquired VitalTech, a provider of comprehensive enterprise solutions across the care continuum.

– The strategic acquisition, advised by Hyde Park Capital, represents CoachCare’s eighth acquisition since 2023 and significantly enhances its virtual care capabilities.

VitalTech Background

VitalTech, founded in 2019, brings unique software and AI expertise to CoachCare’s existing suite of RPM, Chronic Care Management, and other services. The acquisition reinforces CoachCare’s commitment to providing a complete RPM solution, moving healthcare providers beyond technology to deliver comprehensive virtual care.

VitalTech’s Innovative Solutions

VitalTech develops solutions that empower patients to actively manage their health and wellness while enabling care teams to remotely monitor, manage, and care for patients. Its proprietary platform allows health systems, managed care organizations, home health agencies, physicians, and senior living facilities to automate clinical workflows, improve health outcomes, increase patient safety, and reduce the cost of care. VitalTech’s user-friendly solutions are designed to boost patient engagement and compliance.

“This strategic acquisition marks an exciting new chapter for VitalTech’s team and our expanding network of large health system clients. Over the past few years, we have built an innovative, AI-first platform designed to revolutionize virtual care. Joining forces with CoachCare allows us to further scale our solutions, expand payment models, and deliver even greater value to our existing and future clients. Together, we will enhance patient outcomes, empower healthcare providers, and shape the future of virtual care. We have long admired CoachCare’s vision and unwavering commitment to consolidating the virtual care market, and we are thrilled to become part of this rapidly growing company,” said Jeh Kazimi, CEO of VitalTech.

CoachCare’s Comprehensive Virtual Care Platform

CoachCare, based in New York, offers a comprehensive suite of virtual tools, including remote patient monitoring, chronic care management, video conferencing, real-time messaging, screening, content sharing, and connected devices.
