This transition is part of a strategy to accelerate innovation for the Care Management platform. Together, the Care Management and CarePort platforms offer providers, payers and ACOs unparalleled visibility across all levels of care and transitions across the continuum.
Acquired by Allscripts in 2016, CarePort enables providers to enhance post-acute outcomes and costs by guiding patients across the care continuum and tracking their recovery in real-time. It’s EHR-agnostic platform is used by nearly 1,000 hospitals across the country in its network using the tools:
– Care Management – A complete Discharge Planning and Utilization Review workflow with a built-in network of providers.
– Referral Management – An intake, referral management and discharge planning tool for post-acute care providers.
Moving forward, the CarePort suite of products are CarePort Care Management, CarePort Referral Management, CarePort Guide, CarePort Connect and CarePort Insight.

Internally, the combined entity is growing to 200 full-time employees by the end of 2018. The Boston-based team recently moved into a new, larger office space to accommodate its expanding team.
