MiHIN Partners with CarePort Health to Improve Care Quality Statewide

– Michigan Health Information Network Shared Services (MiHIN) is partnering with CarePort Health to lower costs of care, improve care quality and outcomes for Michigan’s state-wide patient population through better care coordination.

– The partnership allows CarePort to leverage data flowing through MiHIN to provide higher quality data to participating payer and provider organizations, enabling more cost-effective management of patient populations. 

CarePort Health, a Boston, MA-based provider of care coordination and post-acute outcomes management has announced its partnership with Michigan Health Information Network Shared Services (MiHIN), Michigan’s state-designated entity to improve healthcare quality, efficiency and patient safety within the state of Michigan.

Optimizing Patient Experience & Outcomes Statewide

The partnership allows CarePort to leverage data flowing through MiHIN to provide higher quality data to participating payer and provider organizations, enabling more cost-effective management of patient populations. MiHIN’s provider organizations also use CarePort’s care coordination platform for real-time data updates and enhanced visibility into patient’s transitions and critical health events, which enables MiHIN to optimize patient experiences and outcomes statewide.

On the regulatory front, the partnership will also help Michigan to capitalize on its 2014 partnership with CMS by allowing state provider and payer organizations participating in MI Health Link, the state’s Dual Eligible Demonstration program, to more easily adhere to program requirements, such as patient follow-up post-discharge.
