CarePort Guide is an interactive tool used by discharge planners, patients, families and care managers to find high-quality post-acute care for a patient’s specific needs. Through the CarePort-maintained, national database of post-acute providers, patients and families have greater visibility into available post-acute quality of care and services, which helps them make informed decisions about their care. Providers can also use CarePort Guide to fulfill regulatory requirements including the Conditions of Participation for discharge planning and the IMPACT Act.
Integration Will Ensure Optimized Discharge Planning
Epic users can now use CarePort Guide seamlessly for more informed care decision-making by allowing discharge planners, patients, families and care managers more visibility into the most appropriate, high-quality post-acute providers available that best serve each patient’s specific needs. Notably, CarePort Guide will also help Epic customers ensure compliance and fulfill regulatory requirements for the Conditions of Participation for discharge planning and the IMPACT Act by enabling seamless patient data sharing directly through the Epic EHRduring transitions.
CarePort Guide Integration Workflow with Epic EHR
Available now in the Epic App Orchard marketplace, discharge planners can access CarePort Guide via a link within the Epic care management module. The interactive tool can be searched and filtered based on available post-acute provider information such as quality, preferred provider networks, geography and available clinical services. CarePort Guide also enables patients and families to review post-acute providers at the bedside, or through text message and email.
