Eden Health Earns NCQA Patient Centered Medical Home Recognition

– Eden Health earns National Committee for Quality Assurance’s (NCQA) Patient-Centered Medical Home (PCMH) recognition.

– PCHM recognition underscores Eden Health’s trailblazing role as a tech-enabled primary care delivery model.

– PCMH is widely regarded as the highest quality recognition that exists for primary care.

Eden Health, which delivers primary care in the workplace, near the workplace and virtually, announced it has earned the National Committee for Quality Assurance’s (NCQA) Patient Centered Medical Home (PCMH) recognition. The result of a year of rigorous evaluation by the country’s premier healthcare quality assurance organization, the recognition is based on Eden Health’s innovative care delivery model that puts patients and their experience first. PCMH is widely regarded as the highest quality recognition that exists for primary care. 

Understanding The PCMH Model

At its core, the PCMH model is an evidence-based framework for moving toward an era of accountable, cost-conscious, coordinated care and away from traditional fee-for-service reimbursements. Patient treatment is coordinated through primary care providers to ensure patients receive necessary care when and where they need it. The model emphasizes enhanced access to care, continuous communication between primary care providers and specialists, and a focus on the patient as a whole person. The PCMH model ultimately aims to transform primary care into a highly patient-friendly model.

The Bigger Picture

Earning PCMH recognition is difficult. And In a marketplace saturated with claims of high-quality healthcare, it’s a road none of One Medical, Carbon Health or Crossover have taken. This might suggest different visions in what a new wave of primary care providers believe primary care should look like and how it should be delivered.

Why It Matters

We believe that highly accessible, high-quality primary care aligned with consumer preferences is the path forward.”

The company recently raised $10M in Series A funding in late February led by existing investor Greycroft. Existing investors PJC, 645 Ventures, Max Ventures and Company participated in the round along with new investors Convene, Aspect Ventures and Brand Foundry.
