Allscripts Care Management Merges with CarePort Health to Build Care Coordination Tools

Allscripts Acquires CarePort

CarePort Health, an Allscripts company and pioneer in post-acute outcomes management, today announced it has assumed management of the Allscripts Care Management solution and services portfolio. This transition is part of a strategy to accelerate innovation for the Care Management platform. Together, the Care Management and CarePort platforms offer providers, payers and ACOs unparalleled visibility across all levels of care and transitions across the continuum.

Acquired by Allscripts in 2016, CarePort enables providers to enhance post-acute outcomes and costs by guiding patients across the care continuum and tracking their recovery in real-time. It’s EHR-agnostic platform is used by nearly 1,000 hospitals across the country in its network using the tools:

– Care Management – A complete Discharge Planning and Utilization Review workflow with a built-in network of providers.

– Referral Management –  An intake, referral management and discharge planning tool for post-acute care providers.

Moving forward, the CarePort suite of products are CarePort Care Management, CarePort Referral Management, CarePort Guide, CarePort Connect and CarePort Insight.

Bridging Care Management and CarePort represents a natural progression given the current industry trends. “The shift to outcomes-based reimbursement models created a heightened need for providers and health organizations to transition patients out of the hospital efficiently with established solutions, such as Care Management, and to track patients’ health as they transition across multiple care settings,,” said Dr. Lissy Hu, chief executive officer and founder of CarePort Health. “This new organization combines the experience and talent of Care Management and CarePort into one team with a singular focus on care coordination and the directive to be nimble and responsive to customer needs.”

 

Along with its strategic partnership with Allscripts, CarePort Health has organically grown its customer base of several leading medical centers, accountable care organizations (ACOs), and hospitals across the United States. Internally, the combined entity is growing to 200 full-time employees by the end of 2018. The Boston-based team recently moved into a new, larger office space to accommodate its expanding team.
