Remo Health and Cleveland Clinic ACO Launch Pilot to Deliver Comprehensive Virtual Dementia Care

What You Should Know: 

– Remo Health, a virtual dementia care provider, and the Cleveland Clinic Medicare Accountable Care Organization (ACO) today announced a pilot initiative to deliver comprehensive support to eligible patients living with dementia and their caregivers. The strategic pilot focuses on improving the quality of care for Medicare patients while lowering costs.

– The collaboration will provide robust resources and personalized care, which is especially crucial for rural families who often drive hours to appointments and for working caregivers. As an added benefit, Cleveland Clinic ACO patients in Ohio will now have access to a nationwide peer support network of 20,000 families, connecting them with others navigating a similar journey.

Addressing the Growing Dementia Care Crisis

Dementia is becoming increasingly prevalent, placing significant pressure on the healthcare system to meet the complex and continuous needs of patients and their caregivers. Approximately a quarter million Ohioans are currently living with dementia, with more than 450,000 individuals serving as unpaid, round-the-clock caregivers. This caregiving role is often isolating and overwhelming, with two-thirds of these caregivers struggling with chronic health conditions themselves, often associated with the daily demands and stress of care.

The Cleveland Clinic ACO is committed to helping its patients age independently in their homes for as long as possible. This partnership with Remo Health supports these efforts and is designed to address the growing needs of Ohioans living with dementia.
