The alliance, which is already underway, is expected to help Change Healthcare expand their capabilities in population healthcare management, close communications gaps between health plans and care providers and assist the company in more actively supporting their primary care physicians, particularly in targeting major chronic conditions. For HGE, the move will place its innovative digital health platform into the hands of likely millions more critical patients and their physicians.
The HGE digital telemedicine platform and mobile app allow physicians to remotely care for a patient from anywhere in the nation, regardless if they are awaiting/recovering from a lung transplant or experiencing other medical issues.
