Carolinas HealthCare System (CHS) has selected Cerner to implement their HealtheIntent population health management platform. The implementation will coordinate and manage care for its more than 12 million patient interactions each year across its health system and clinically integrated network, Carolinas Physician Alliance (CPA). Additionally, CHS will continue to use the Cerner Millennium® integrated EHR and will also extend its remote hosting relationship with Cerner.
HealtheIntent’s system-agnostic platform normalizes data from any source, to aggregate clinical and financial data from its core EHR and other sources. Once live, HealtheIntent will enable thousands of CHS clinicians to make more informed decisions by providing a comprehensive profile of an individual and the entire population. This comprehensive profile can be used to proactively identify gaps in care and analyze population and enterprise data through use of HealtheRegistries, HealtheEDW℠, Cerner’s registries and scorecards and enterprise data warehouse solutions.
