Transcend Insights®, a population health health-focused subsidiary of Humana, today announced the launch of a unified solution suite, built on its technology platform HealthLogix™. The new offering, which expands upon Transcend Insights’ population health platform for community-wide interoperability and real-time analytics, will provide health system executives, care teams and patients with a full suite of solutions designed to help them transition from episodic to proactive care for long-term wellness.
Transcend Insights’ unified solution suite includes:
– HealthLogix Populations™ – A performance measurement application that provides actionable, configurable visualizations and dashboards to help health system executives and care teams quickly evaluate quality and utilization metrics, and compare performance across an entire care community. Users can access comprehensive data visualizations, such as facility performance comparison views to identify opportunities to improve health outcomes.
– HealthLogix Care – A tablet and web-based application that provides physicians and care teams with a single, comprehensive view of a patient’s clinical, claims and wellness data at the point of care. Complementing electronic health records, this solution generates timely, proactive insights that support confident decision making for a more engaging patient care experience.
– myHealthLogix – Powered by Patient IO, a leading care coordination platform, myHealthLogix is a patient engagement solution that connects care teams with their patients by providing collaborative care plans, secure messaging, medication management capabilities, and tailored educational information to enable a more coordinated approach to care.
– HealthLogix Market – A future offering that will establish an open, scalable marketplace for third-party health care applications developed on the HealthLogix platform. HealthLogix Market will enable access to valuable data and insights via Fast Healthcare Interoperability Resources (FHIR) Application Programming Interfaces (APIs) to add another level of technical customization and increased benefit to existing HealthLogix platform clients and partners. Patient IO is among the first HealthLogix Market partners. In addition, Transcend Insights has partnered with Chiron Health, an organization that provides secure video visit technology and Medisas, a workflow and communication platform for hospitals.
