Advance Health Raises $40M to Expand In Home Health Assessments

To improve healthcare, the next evolution of member care is necessary. In a study by The Physicians Foundation, 27 percent of Americans do not have a Primary Care Physician (PCP) and of those that do, 22 percent do not visit their PCP on a regular basis. Patients fail to follow care plans and medication guidelines, or do not to take the basic steps to care for themselves. The result: members are not engaged, care that is fragmented and providers and health plans do not have a complete picture of the member’s health.

This week, Advance Health, a provider of managed care prospective health assessments and care management solution announced it has raised $40 million in minority growth equity investment led by Summit Partners, with participation from Noro-Moseley Partners. The managed care leader will utilize the capital to continue product innovation, broaden market awareness, and expand its sales and services capabilities.

Advanced Member Engagement

Advance Health engages managed care members where they are most comfortable by providing in-home health risk assessments and chronic care management services through its extensive provider network. Advance Health is the first company to completely capture health assessments electronically through our proprietary iOS iPad and HTML5 applications. The mobile app provides client level customization with built-in intelligence that triggers questions based on data input from a member’s response or exam results and provides customized recommendations and care plans specifically for that member. This approach to health assessments enables nurse practitioners (NPs) to collect more specific member information, clearly assess their care needs and utilize workflow technology to close care gaps.
