Caradigm & Beacon Partners Collaborate to Deliver Solution for Population Health Management

Caradigm, a population health company, and Beacon Partners, a leading healthcare management consulting firm, today announced an agreement to deliver a comprehensive solution for improving population health management and transitioning to value-based care. The agreement joins Caradigm’s population health software and the healthcare consulting expertise of Beacon Partners in order to help providers manage risk, improve care quality and outcomes, and reduce the per capita cost of care. Under the agreement, the companies will collaborate to help healthcare providers accelerate the deployment and adoption of Caradigm’s portfolio of healthcare analytics and population health solutions, including Caradigm™ Risk Management and Caradigm™ Care Management. As a critical part of this effort, Beacon Partners will offer advisory services, clinical and IT transformation, and implementation services for Caradigm customers. These services are designed to help organizations understand value-based care opportunities, assess their readiness to shift successfully to risk-bearing payment models, and develop comprehensive transition and implementation plans. “Succeeding in population health and value-based care requires significant transformation on the part of providers, who are increasingly taking on more risk,” said Michael Simpson, CEO of Caradigm. “Beacon Partners brings proven experience in helping organizations navigate these changes.
