What You Should Know
- Seattle-based virtual respiratory care provider Kivo Health has raised a $10M Series A financing round led by Allumia Ventures.
- The round included participation from Corewell Health Ventures, Y Combinator, Heuristic Capital, and MassMutual Ventures, bringing Kivo’s total institutional funding to just over $16M.
- The new capital provides the company with approximately two years of operational runway, with commercial market expansion expected as early as next year.
- Kivo Health specializes in delivering virtual pulmonary rehabilitation and continuous respiratory management for senior patients diagnosed with chronic obstructive pulmonary disease (COPD).
Home-Delivered Pulmonary Rehabilitation: Cellular Hardware and W-2 Clinicians
Kivo Health focuses specifically on vulnerable senior populations diagnosed with moderate-to-severe COPD, shifting pulmonary rehabilitation out of brick-and-mortar outpatient facilities and directly into the home:
- Turnkey Hardware Fulfillment: Rather than expecting elderly patients to navigate consumer app stores or maintain home Wi-Fi configurations, Kivo ships patients a dedicated, cellular-connected tablet, resistance exercise bands, and a Bluetooth pulse oximeter.
- Front-Loaded Clinical Intensity: Patients begin with an intensive, two-month clinical onboarding phase, engaging in bi-weekly live video appointments with licensed respiratory therapists and exercise physiologists. During these encounters, clinicians guide patients through structured aerobic and strength conditioning while monitoring continuous pulse oximetry, validating inhaler administration techniques, and teaching early symptom recognition protocols.
- Longitudinal Maintenance Phase: Following the initial two-month program, patients step down to monthly monitoring visits for at least eight months, establishing a year-long clinical safety net designed to intercept acute flare-ups before they result in hospital boarding.
- Employed Care Delivery Bench: Unlike transactional telehealth platforms that rely entirely on 1099 contractor gig models, Kivo directly employs its core clinical team of respiratory therapists and exercise physiologists. Supervising board-certified pulmonologists operate through a hybrid model of direct employment and specialized medical director contracts.
From Fee-for-Service Entry to Medicare Advantage Capitation
Kivo’s commercial growth strategy leverages health systems’ existing clinical capacity while preparing for two-sided value-based risk:
- Health System Partnership Channels: The majority of Kivo’s current business flows through strategic health system partnerships. Under this fee-for-service model, hospital pulmonology and internal medicine clinics refer post-discharge COPD patients into Kivo’s virtual clinic, allowing health systems to extend clinical capacity, reduce 30-day COPD readmission penalties under the Hospital Readmissions Reduction Program (HRRP), and capture downstream remote physiologic monitoring (RPM) and remote therapeutic monitoring (RTM) billing codes.
- Scaling Risk-Bearing Medicare Advantage Alignment: In parallel with fee-for-service hospital referrals, Kivo is actively scaling value-based arrangements directly with Medicare Advantage organizations and accountable care organizations (ACOs). By preventing avoidable emergency room visits and inpatient admissions—which average $19,000 or more per COPD hospitalization—the platform generates substantial medical cost offsets, allowing Kivo to participate in shared savings and risk-adjusted capitation benchmarks.
