- WebMD Ignite, CCS Health, and Medicine for the Greater Good (a Johns Hopkins Medicine initiative) launched the Collaborative Care Network™ (CCN), a clinical-community operating platform for rural health systems.
- Aligns with the Centers for Medicare & Medicaid Services (CMS) Rural Health Transformation Program, which is distributing $50 billion between 2026 and 2030 to expand rural access, workforce, technology, and regional healthcare partnerships.
- Addresses major geographical disparities: 63% of designated primary-care shortage areas are rural (HRSA), and adults in nonmetropolitan areas are more than twice as likely to rate local healthcare access as fair or poor compared to urban residents (AHRQ).
- Integrates three foundational capabilities: WebMD Ignite’s clinical-grade digital content, risk assessments, and HEART program; CCS Health’s care coordination, workforce training, and Community Health Record™; and Medicine for the Greater Good’s community-clinician engagement model.
Building Connected Infrastructure for Rural Health
The launch coincides with states rolling out the Centers for Medicare & Medicaid Services’ (CMS) Rural Health Transformation Program, a $50 billion federal initiative deploying from 2026 through 2030 to bolster rural clinical access, workforce development, and prevention.
Rather than imposing top-down programs, CCN connects existing community assets through six core capabilities:
- Community Access Points: Delivers clinical support and education directly to schools, workplaces, places of worship, and community centers.
- Workforce Integration: Trains and deploys community health workers and care navigators to manage chronic diseases, maternal health, and care transitions.
- Shared Health Records & Analytics: Uses CCS Health’s Community Health Record™ and analytics to manage closed-loop social referrals and track performance metrics.
- Continuous Patient Engagement: Leverages WebMD Ignite’s clinical content, digital risk assessments, and remote patient monitoring between doctor visits.
Founding Partner Contributions
- WebMD Ignite: Supplies clinical education libraries, digital engagement tools, and risk assessments through its HEART (Health Education & Access for Rural Transformation) program.
- CCS Health: Contributes care coordination platforms, the Community Health Record™, and workforce training frameworks.
- Medicine for the Greater Good: Provides community-clinical partnership models developed at Johns Hopkins Medicine to integrate social determinants of health into standard chronic care workflows.
