  • Expanding Rural Primary Care Reach: Integrates Homeward’s rural-first clinical model—co-founded by Dr. Jenny Schneider and Amar Kendale—incorporating regional health plan partnerships (including Blue Cross Blue Shield of Michigan) and nearly 50,000 rural members into Cityblock’s national footprint.
  • CORE (Care Orchestration and Resourcing Engine): Expands Cityblock’s proprietary operating platform across the combined population. Rather than relying on generic risk tiers, CORE leverages nearly a decade of longitudinal member data to predict the specific clinical or social intervention required to resolve an individual care gap, achieving validated prediction accuracy rates between 62% and 87%.
