Initiated on July 1, 2026, the collaboration focuses on Humana Dual Eligible Special Needs Plan (D-SNP) beneficiaries, individuals with partial dual eligibility, and Medicare Advantage Low-Income Subsidy (LIS) recipients who manage complex chronic conditions.
Dual-eligible beneficiaries routinely struggle with fragmented healthcare systems, navigating conflicting Medicare and Medicaid benefit structures that frequently lead to missed preventive care, uncoordinated social supports, and avoidable emergency department utilization. Cityblock’s model addresses these care gaps by embedding multidisciplinary care teams directly into the member’s daily environment.
The North Carolina deployment pairs Cityblock’s care delivery infrastructure with Humana’s regional health plan operations:
