Assort Health Launches AI Agent to Automate End-to-End Referral Conversion

Manual intake workflows require staff to manually extract data, verify insurance, and initiate outreach, resulting in multiday delays during which patients seek care elsewhere or drop off entirely.

Assort Health’s Referrals agent replaces passive queue management with an autonomous execution model:

  • Multichannel Ingestion & Extraction: Automatically ingests and structures incoming referral documents from EHRs, faxes, scanned PDFs, and phone records.
  • Synapse AI & Provider Matching: Powered by Assort’s proprietary Synapse engine (trained on over 225 million specialty-specific patient interactions and 70,000 care protocols), the agent evaluates clinical complaints and payer criteria. In 50% of referrals, the AI identifies an optimal, better-fit provider than the one originally named.
  • Proactive Multichannel Scheduling: Reaches out to patients via voice, two-way text, or email to book appointments directly into the EHR within the hour of referral receipt.
  • Continuous Journey Memory: Maintains contextual interaction memory across channels. If a patient re-engages days later via phone or text, the AI resumes the conversation without forcing the patient to restart intake.
  • Closed-Loop Provider Updates & Triage Escalation: Automatically notifies referring physicians when a referral is scheduled, completed, or deemed ineligible, while escalating complex clinical cases to human staff with attached source documentation.
