As remote patient monitoring (RPM) and Chronic Care Management (CCM) expand across value-based care networks, health systems face a critical constraint: continuous cellular-enabled biometric data streams generate high volumes of false alerts. Care teams are often overwhelmed by raw alert noise, leading to clinician burnout, alert fatigue, and delayed interventions. Without automated data triage and EMR-integrated software orchestration, scaling primary care and chronic condition management across high-acuity patient cohorts becomes operationally unsustainable.
