According to the latest data from ECRI, the nation’s leading independent patient safety organization, the unchecked proliferation of AI chatbots has become the single greatest technology hazard facing patients today.

The allure is undeniable. With over 40 million people turning to platforms like ChatGPT daily for health information, the barrier between patient and medical advice has dissolved. However, ECRI’s Top 10 Health Technology Hazards for 2026 report suggests that this accessibility comes at a steep price: the erosion of accuracy in favor of algorithmic confidence.

The Technical Hazard: “Expert-Sounding” Hallucinations

ECRI warns that chatbots rely on large language models (LLMs) that predict word patterns rather than understanding medical context. This can lead to highly confident but dangerously false information:

  • Medical Inventiveness: Chatbots have suggested incorrect diagnoses, recommended unnecessary tests, and even invented body parts while sounding like a trusted expert.
  • Dangerous Clinical Advice: In one ECRI test, a chatbot incorrectly stated it was appropriate to place an electrosurgical electrode over a patient’s shoulder blade—a mistake that would cause severe patient burns.
  • The “Context” Problem: Because these models satisfy users by providing any answer, they lack the ability to replace the expertise and experience of human professionals.

Socioeconomic and Equity Risks

The report highlights that the risks of chatbot reliance are compounded by broader systemic issues:

  • The Substitute Care Model: As healthcare costs rise and clinics close, more patients may rely on chatbots as a substitute for professional advice, increasing the likelihood of unvetted, harmful decisions.
  • Entrenching Disparities: AI models reflect the biases embedded in their training data. If not carefully managed, these tools can reinforce stereotypes and inequities, entrenching disparities that health systems have worked for decades to eliminate.
