The program brings together individualized coaching, group learning, condition-specific curricula, GLP-1 education, CGM data review, and digital tracking tools so patients can build durable routines that improve overall metabolic health.
South Carolina’s need is significant. The state records roughly 25,000 new diabetes diagnoses each year, with 13 percent of adults living with diabetes and 35 percent living with obesity.
