Aeroflow Health and Molina Launch Virtual Diabetes Care Program in South Carolina

– Aeroflow Health has launched a statewide partnership with Molina Healthcare of South Carolina to expand access to virtual nutrition counseling and diabetes self-management education (DSME) for Commercial and Medicare members.

– The initiative is built to dismantle persistent access barriers by offering evidence-based metabolic and nutrition care at little to no cost, delivered entirely through Aeroflow’s virtual infrastructure.

Virtual program expands access to metabolic and nutrition care statewide

Under Aeroflow’s integrated Nutrition Therapy Program, members are connected to registered dietitians and certified diabetes specialists who provide tailored metabolic support—from blood-glucose management and lifestyle coaching to long-term risk-reduction strategies. The program brings together individualized coaching, group learning, condition-specific curricula, GLP-1 education, CGM data review, and digital tracking tools so patients can build durable routines that improve overall metabolic health.

South Carolina’s need is significant. The state records roughly 25,000 new diabetes diagnoses each year, with 13 percent of adults living with diabetes and 35 percent living with obesity. These conditions carry steep health and financial burdens, with people living with diabetes incurring medical costs more than 2.5 times those of individuals without the disease. This often restricts their ability to seek ongoing preventive care.

By minimizing out-of-pocket costs and removing geographic barriers, the partnership aims to strengthen preventive care and chronic-disease management statewide. Early engagement data suggest that structured nutrition and DSME programs lead to improvements in energy, sleep, mood, and disease-specific outcomes, while lowering the risk of downstream complications associated with diabetes, obesity, and cardiovascular disease.

Amanda Minimi, Vice President of Health Solution Marketing & Operations at Aeroflow Health, emphasized that nutrition services often serve as a lifeline for patients navigating complex metabolic challenges. She noted that by expanding access to these programs, health plans can play a more active role in supporting informed decision-making and long-term wellness.

Aeroflow’s virtual delivery model ensures that Molina members can access services regardless of location or transportation limitations. Educational modules cover mindful eating, heart-healthy nutrition, diabetic weight loss, and medication-assisted management, enabling members to build personalized plans aligned with their day-to-day lives.
