Sage Care Launches with $20M to Deliver AI-Powered Care Navigation and Boost Revenue

– Sage Care, a digital health platform designed to eliminate healthcare navigation inefficiencies, today announced its public launch, backed by $20M in funding led by Yosemite, with continued support from General Catalyst, Metrodora (Chelsea Clinton), and other investors, 

– The funding cements the company’s vision to reinvent the healthcare front office using clinically intelligent AI agents.

Solving the Healthcare Navigation Crisis

The foundation of Sage Care is the recognition that millions of Americans struggle to navigate the healthcare system, often failing to reach the right provider at the right time. The company aims to solve this complex routing and scheduling issue by applying its technical expertise in automation and AI systems.

“We’re bringing our technical expertise in routing, automation, and complex AI systems to healthcare with the hopes of helping as many people as possible get the care that they deserve,” said Justin Ho, CEO of Sage Care.

The company was founded in 2024 by second-time founders, including Justin Ho (co-founder of rideOS and a leader of Uber’s self-driving division), Dr. Caesar Djavaherian (co-founder of Carbon Health), and Chris Blumenberg (who worked on the original iPhone), after experiencing the frustration of the current healthcare system.

AI Agents for Optimized Care Delivery and Financial Impact

Sage Care’s platform utilizes advanced optimization algorithms and medically informed AI agents to streamline administrative and patient interactions.

The platform has demonstrated the potential to help health systems achieve a 15–20% revenue increase through optimized care coordination, reduced no-shows, and improved throughput.

Key capabilities of the AI agents include:

  • 24/7 Triage and Support: Answering and triaging patient calls, messages, and requests around the clock.
  • Administrative Management: Managing appointment scheduling, referral processing, and insurance verification.
  • Care Coordination: Coordinating follow-ups for diagnostics, care plans, and medication adherence.
