A Proven Program with Measurable Impact

The collaboration is a direct response to the growing diabetes epidemic in the U.S. More than 40 million people live with diabetes, and nearly one in four of them don’t even know they have it. The unmanaged condition can lead to heart disease, stroke, vision loss, and premature death, costing the country over a trillion dollars a year in direct medical costs and lost productivity.

The CARES Program focuses on helping those who are often left behind, such as the uninsured, underinsured, isolated seniors, and families under financial duress. The program’s impact is significant, with every $1 invested yielding up to $24 in avoided costs and improved outcomes.

One success story is a recipient named Artimas. In just three months with the CARES Program, Artimas lowered his A1c from 11.6% to 6.5%, lost 24 pounds, and controlled his blood pressure. An A1c reduction of this magnitude has a powerful effect on lowering the risk of serious complications:

  • Stroke: -25%
  • Kidney Failure: -33%
  • Limb Loss: -43%
  • Early Death: -21%
