Interwell Health Launches Nephrology Rounds App with Epic EHR Integration

– Interwell Health, a value-based kidney care management company, has unveiled Acumen Rounder, a nephrology rounding application that seamlessly integrates with Epic’s  electronic health record (EHR) system. 

– The innovation is set to transform how nephrologists manage patient workflows and deliver value-based care.

Enhancing Nephrology Workflows

Acumen Rounder offers a comprehensive suite of features designed to improve efficiency, accuracy, and the user experience for nephrologists conducting rounds in hospitals and dialysis centers. Key features include:

  • Patient Tracking: Enables easy identification of patients needing attention and their location, with flags for those in value-based care arrangements.
  • Hospital Follow-Up: Facilitates seamless communication of follow-up needs for hospitalized patients to office staff, improving care transitions.
  • Transitional Care Management (TCM): Automates workflows for managing recently discharged dialysis patients, including outreach, documentation, and billing for TCM visits.
  • Clinical Documentation: Supports efficient dialysis note entry and consolidated monthly capitated billing (MCP).
  • Predictive Text: Streamlines data entry with predictive text for diagnoses, enhancing documentation speed.
  • Modern Interface: Offers a user-friendly and intuitive design for an enhanced user experience.
  • Wide Integration: Seamlessly integrates with Acumen Epic Connect and the clinical networks of leading dialysis companies.

Replacing Legacy Systems

Acumen Rounder will replace Interwell Health’s legacy application, Acumen Mobile Charge Capture, with an anticipated user base of over 2,300 providers in 138 nephrology practices by the end of 2024.
