– Health Commons Project, a non-profit dedicated to improving healthcare access and quality, has acquired OneHealthPort, Washington State’s Health Information Exchange (HIE). 

– The strategic move strengthens Health Commons Project’s mission to enhance data interoperability, improve care coordination, and promote public health across Washington.

Expanding Impact and Reach

By integrating OneHealthPort’s robust HIE platform, Health Commons Project gains a powerful tool to connect healthcare providers, health systems, and public health agencies with secure, real-time access to patient data. This acquisition builds upon OneHealthPort’s established network, which serves over 35,000 healthcare entities and 150,000 individuals in Washington.

Key Benefits for Washington’s Healthcare Ecosystem

This acquisition brings several key advantages:

  • Enhanced Data Interoperability: Connecting a wider range of health and social care providers, including fire departments, schools, rural pharmacies, and community-based organizations, ensuring critical patient data is available when and where it’s needed.
  • Improved Care Coordination: Streamlining patient transitions between different care settings, leading to better outcomes and reduced costs.
  • Support for Public Health Initiatives: Providing real-time, de-identified data insights to public health agencies to enhance their ability to respond to crises and monitor population health trends.
  • Increased Accessibility: Offering new, affordable ways for providers with limited IT resources to access critical health data and reports.

A Seamless Transition and Future Enhancements

Health Commons Project and OneHealthPort are committed to ensuring a smooth transition for users. No significant disruptions to services are expected when OneHealthPort officially becomes HCP OneHealthPort, LLC, on January 1, 2025.

Health Commons Project plans to enhance the HIE platform with new features and tools to improve functionality and accessibility. This commitment to innovation will further empower healthcare stakeholders across Washington State.
