What You Should Know:

– Form Health, the leading provider of science-based obesity care in the United States raised $38M in Series B funding led by Sound Ventures, the round also included participation from M13, SignalFire, and Next View Ventures.

– By combining science-based treatment plans with a convenient telehealth platform and a focus on building a team of top specialists, Form Health aims to make effective and personalized weight loss solutions accessible to a wider population.

Investing in Growth: Three Key Pillars

Form Health will leverage the funds to propel its growth in three key areas:

  • Enterprise Partnerships: Form Health has already established partnerships with major employers, including Fortune 200 companies. They plan to build on this success by expanding their reach to additional employers, consultants, and health plans.
  • Scaling the Team with ABOM-Certified Physicians: Form Health prioritizes personalized care delivered by board-certified specialists. There’s a limited pool of ABOM-certified obesity medicine physicians in the US (around 8,000), and Form Health will invest in recruiting and training more to meet patient demand.
  • Technology and Product Development: Telehealth is at the core of Form Health’s model. They will continue to invest in their virtual platform, including telehealth visits, unlimited messaging with care teams, and tools like digital scales and interactive apps to optimize remote patient care delivery.

Science-Based Telehealth for Weight Loss

Founded in 2019 by Evan Richardson (CEO) and led by Chief Medical Officer Dr. Florencia Halperin (co-founder of the Center for Weight Management and Metabolic Surgery at Brigham and Women’s Hospital), Form Health has provided care to tens of thousands of patients across the US through their unique telehealth model. This model offers patients:

  • Dedicated Care Teams: Each patient is assigned a team consisting of a board-certified physician and a registered dietitian.
  • Personalized Treatment Plans: Care teams develop individualized weight loss plans addressing lifestyle changes and incorporating FDA-approved weight loss medication when appropriate.
  • Accessibility and Convenience: Form Health is in-network with most major insurance plans, including Medicare, and allows patients to participate through employer programs or by signing up individually.

“We are in a transformational moment in how we understand and treat obesity and metabolic disease and, with this new funding, Form Health can accelerate our work setting the standard in safe, high-quality, evidence-based obesity care,” said Evan Richardson, founder and CEO of Form Health. “Form Health’s many strategic relationships with employers, pharma companies, and patients are the result of offering science-based obesity care delivered by board-certified physicians and Registered Dietitians that drives long-lasting results for patients. This approach is fundamentally different from the many players in the category who are focused on handing out GLP-1s or potentially dangerous compounded medicine without the clinical care required for safe, long-term patient outcomes. We are excited to continue to deliver exceptional obesity care to the many Americans who need it.”
