M&A: BehaVR and Fern Health Merge to Form RealizedCare Focused on Digital Therapeutics

– BehaVR and Fern Health have merged to create RealizedCare, a new company that will provide digital care management solutions for chronic pain. RealizedCare’s platform combines digital therapeutics (DTx) with traditional wraparound services to deliver personalized care at scale.

– The company has partnered with GrĂ¼nenthal, a global research-based pharmaceutical company, as a strategic investor.

Chronic Pain Challenges

Chronic pain affects an estimated 52 million adults in the U.S., driving substantial healthcare costs. RealizedCare aims to disrupt care delivery, offering a personalized and integrated approach to managing chronic pain. Fern Health’s chronic pain programs, initially developed with Massachusetts General Hospital, focus on holistic, multi-modal care. The merger combines Fern Health’s virtual offering with BehaVR’s expertise in building immersive, evidence-based DTx, enhancing engagement and impact across RealizedCare’s therapeutics suite.

Engagement with Health Plans and Providers

RealizedCare will collaborate with health plans, employers, and value-based providers, offering a fully managed solution to identify, assess, and engage members with chronic pain. The platform guides individuals through a personalized care journey, incorporating digital interventions, community support, and direct clinician referrals when necessary.

RealizedCare’s offering includes assigning a pain coach to members, delivering personalized care plans comprising pain neuroscience education, mindfulness practices, calming experiences, and immersive, gamified, graded exercise and movement. Members gain access to PatientsLikeMe’s community of over 160,000 individuals living with chronic pain, providing peer and community support, enriching the care experience.
