– Blue Cross Blue Shield of Kansas City announces their collaboration with Wellframe, a leader in digital health management, to bring its health plan members into a new era of health management.

– Through Wellframe’s innovative app, Blue KC members can access digital resources and support for chronic and complex conditions, wellness, transitions of care, maternal health, cancer, and end-stage renal disease.

– The Wellframe solution helps members build trusted relationships with Blue KC’s care teams which enable early interventions.

Wellframe, Inc., today announced Blue Cross and Blue Shield of Kansas City will offer Wellframe’s digital health management solution to support its members beyond the walls of care delivery. This new initiative demonstrates Blue KC’s continued commitment to industry-leading support for members’ holistic needs. By delivering digital resources and support for chronic and complex conditions, wellness, transitions of care, maternal health, cancer, and end-stage renal disease, the Wellframe solution helps members build trusted relationships with Blue KC’s care teams which enable early interventions.

Impact of Collaboration

Blue KC has long been a leader in industry-care and the investment in the Wellframe technology marks the future of medical care. The custom-daily digital health app will offer better round-the-clock support for BCBS members, and further the effectiveness of Wellframe’s technology. By engaging members in these programs, Wellframe facilitates high touch, sustained, and supportive relationships between members and their care team through secure mobile messaging.

Wellframe’s patented digital health management solutions extend the reach and effectiveness of Blue KC’s care team by delivering clinical programs to members’ smartphones or tablets in the form of a customized, interactive daily health checklist.

“One of our biggest goals is to make it easier for our members to get the care they need when they need it,” said Raelene Knolla, Vice President of Population Health for Blue Cross and Blue Shield of Kansas City. “We believe the ability for our nurses to interact directly with members digitally through the devices so many members have in their hands daily will significantly increase the health and wellness of our members.”
