HGE Health, Change Healthcare Partner on Value-based COPD Care Transformation

HGE Health, a healthcare technology company that works worldwide with pulmonary surgeons, physicians and patients, is joining with healthcare information technology giant Change Healthcare to rapidly accelerate the industry’s evolution to value-based care for COPD, pulmonary issues and a host of other critical diseases.

 

The alliance, which is already underway, is expected to help Change Healthcare expand their capabilities in population healthcare management, close communications gaps between health plans and care providers and assist the company in more actively supporting their primary care physicians, particularly in targeting major chronic conditions.  For HGE, the move will place its innovative digital health platform into the hands of likely millions more critical patients and their physicians.

The HGE digital telemedicine platform and mobile app allow physicians to remotely care for a patient from anywhere in the nation, regardless if they are awaiting/recovering from a lung transplant or experiencing other medical issues.  Thanks to a daily digital interaction with their healthcare team, the mobile app provides patients peace of mind that, pre-surgery, they stand an increased chance of keeping their health stable and remaining on a lung transplant waiting list.  More importantly, post-surgery, or in simple health support situations, patients benefit from daily connectivity with their care team, ensuring the patient has the best chance to avoid post-procedure complications and readmission or to simply better maintain their health.
