HealthPartners and Welltok has announced a partnership that will allow HealthPartners to deliver a more personalized health experience for its members. The new health personalization platform will allow participants to create customized health goals (for example, eating less salt or exercising more), and then get rewarded for meeting those goals.
The technology will also suggest programs and resources that address emotional and financial health. It will also be linked to their individual health plan, allowing them to easily get insight into things like the cost of an ER visit. HealthPartners selected Welltok, recognizing that an experience is most effective when it’s tailored to each person. That means taking into account things like what motivates them, their age, gender, health status and preferences.
Because the technology is linked to their health plan, participants can get the most out of their health plan benefits. For example, getting insight on what it would cost to visit the emergency room, or where to go for the most affordable care based on their specific plan.
HealthPartners is rolling out the new platform to select individual and small group plan members this month. They’ll have a greatly improved online and mobile experience. HealthPartners will extend the platform to commercial large group markets in 2018.
“At HealthPartners, we want to help people live healthier. We know that starts with a meaningful, personalized experience,” said Joel Spoonheim, director, Health Promotion, HealthPartners. “Partnering with Welltok helps us take what we’ve already been doing to the next level. We’ll be able to easily and effectively support participants based on their specific needs or goals.”
Welltok’s CaféWell platform organizes the growing spectrum of health and condition management programs, apps, communities and tracking devices and then creates an actionable Personal Health Itinerary™ for each consumer based on their health status, benefits and goals. The platform leverages social connectivity, gaming mechanics and IBM Watson’s cognitive technologies to drive engagement and behavior change.
