Nashville-based healthcare technology company Medalogix has added a product called Touch to its toolkit of predictive analytic workflow solutions. Touch identifies at risk patients and then enhances and automates a home health clinical team’s patient touchpoints. The application’s predictive risk ranking and automated calling features enable home health agencies to better identify and manage their most at-risk patients and complete more fully reimbursable episodes without transfers.
Touch Overview
Touch leverages analytics to predict which patients are most at risk of transferring off a home health agency’s census (for example, to return to the hospital) before the patient’s 60-day care episode is complete. After identifying these high-risk patients, Touch’s automated call scheduler helps clinicians increase patient touchpoints and intervene before a frustrating and costly patient transfer occurs.
By enabling care-episode completion, Touch keeps patients on census and helps reduce:
– LUPA episodes
– Hospital readmissions
– Clinician turnover
– Costs associated with in-house manual calls
“Care happens during touchpoints,” said Medalogix’s CEO Dan Hogan. “Our technology helps ensure the right touchpoints happen with the right patients at the right time. When those factors align, care improves and costs decrease.”
After using Touch’s risk ranking and interactive voice response (IVR), a home health user experienced a 5 percent reduction in transfers to an inpatient facility (TIFs). In total, they avoided more than 50 transfers over a seven-month period. Users also saw a greater improvement in low utilization payment adjustment (LUPA) reduction compared to other agencies without a fully deployed Medalogix solution.
