Care Continuum Alliance Changes Name to Population Health Alliance

Care Continuum Alliance will now be known as the Population Health Alliance (PHA).

After representing population health, disease management, and wellness and prevention stakeholders for over a decade, the Care Continuum Alliance proudly announces a name change, accompanied by the launch of its newly revamped website. The organization will now be known as the Population Health Alliance (PHA), which better reflects the nature of the work our member organizations are doing in this growing and increasingly important field.

“Our organization exists to advance the evolution of population health management,” said Fred Goldstein, PHA Interim Executive Director and Chair of the Board. “Our goal continues to be to form a broad community that brings together all those innovating and delivering new approaches and technologies in order to support and enhance the success of our stakeholders. This name change was inspired by input from our membership, as an acknowledgement of the growing acceptance of population health management as one of the central strategies now being employed across the care continuum to improve quality of care while reducing avoidable healthcare costs.”

The Population Health Alliance achieves its mission by PROMOTING the evolution of the principles of population health and the innovative practical solutions of its members; CONVENING the industry to create a home for stakeholders; FOSTERING LEARNING AND APPLIED RESEARCH among stakeholders to facilitate innovation, establish best practice development and foster positive change of the industry; and ADVOCATING for the adoption of population health principles and the interests of our members to support and advance their success and those of the industry we serve.

“This organization has a history of bringing together organizations and individuals who are passionate about measurably improving the health of populations throughout the entire healthcare system,” said Christobel Selecky, Principal of Population Healthcare Strategies and Chair-Elect of PHA’s Board of Directors. “Our membership reflects the diversity of this field and comprises organizations with deep experience and knowledge in providing population health management interventions along with companies that are designing and developing innovative tools, technologies, and approaches that will continue to improve our field’s efficiency and effectiveness. This unique combination enables the Population Health Alliance to provide thought leadership for all who seek to learn and share in this field.”

“We applaud this transformation as a clear symbol of where we know health care is going,” said David B. Nash, MD, MBA, Dean of the Jefferson School of Population Health and member of PHA Board of Directors. “The new, more inclusive name brings to the fore the work the organization has already been doing in striving to improve health outcomes across populations through individually-targeted, evidence-guided solutions and support implemented across an evolving spectrum of care delivery and community settings, operating approaches, and payment models.”

The new website, http://www.populationhealthalliance.org, offers easy access to key information and services for members and the public at large, and aims to provide a more comprehensive understanding of the organization’s research initiatives, advocacy efforts, and opportunities for collaboration as well as of its members’ role in the transformation of health care delivery. An important new feature is the Population Health Management Glossary, a comprehensive compendium of the most used terms in the industry, which will continue to be updated and improved with our members’ input.

“Our new website complements the organization’s renewed focus, captured in the theme of our annual conference, PHA Forum 2014, focusing on collaboration and communication as the keys to engagement, across all industry segments,” said Goldstein.
