Allscripts Acquires CarePort

Allscripts has acquired CarePort, a Boston-based company that unlocks data from post-acute care providers to better manage care across the continuum. Due to the shift in healthcare reimbursement and legislation focused on delivering critical outcomes, managing post-acute care is now a necessary and integral component of population health strategy.

The acquisition of CarePort’s post-acute outcomes management platform will expand the capabilities of Allscripts comprehensive population health management platform. As part of the acquisition, CarePort will continue to be based in Boston operating as a subsidiary of Allscripts. Financial terms of the acquisition were not disclosed.

Founded by a team of Harvard physicians, CarePort enables providers to enhance post-acute outcomes and costs by guiding patients across the care continuum and tracking their recovery in real-time. CarePort has three key EHR-agnostic solutions:

– CarePort Guide, which facilitates efficient post-acute care selection and helps patients choose high-quality care that is best for their needs;

– CarePort Connect, which manages patients across post-acute care settings with real-time data and alerts;

– CarePort Insights, which evaluates patient outcomes and post-acute provider performance metrics.

CarePort will integrate with Allscripts CareInMotion, a population health management platform that helps healthcare organizations better deliver patient-centered and value-based care. The adaptable framework that helps organizations address their specific population health priorities, such as connectivity, care coordination, analytics and patient engagement.

“By using both CarePort and Allscripts Care Management™, I know how the two solutions can improve post-acute outcomes, and I’m excited to see the two organizations come together. The level of transparency and real-time information the solutions provide regarding our patients is a game changer,” said Alex Alvarez, Vice President for Care Management and Post-Acute Transitions, Maimonides Medical Center. “Allscripts Care Management offers an extensive network of post-acute providers, enabling case managers to send referrals anywhere in the country. Post-discharge, CarePort lets us follow the patient’s journey through post-acute care to collect outcomes data about patients and providers. The combined solution is exactly the kind of patient-centered tool we need to support value-based care and deliver better outcomes.”
