Continuous 100% Encounter Review and Payer-Specific Risk

Rather than relying on spot checks or manual worklists, Phare UM integrates clinical criteria with longitudinal reimbursement data:

  • Comprehensive Case Evaluation: Evaluates every patient encounter across the hospital stay using Phare Intelligence, identifying documentation deficiencies and clinical risk while the patient is still admitted and notes can be clarified.
  • Payer Atlas Risk Calibration: Integrates payer-specific contract terms, local medical necessity policies, and historical denial trends to rank cases by audit likelihood and denial vulnerability, prioritizing worklists based on true financial and clinical risk rather than admission timestamp.
  • Quantified Inpatient vs. Observation Scenarios: Leverages R1’s database of historical payer outcomes to project audit outcomes. In an early customer assessment, Phare UM identified that approximately 65% of observation encounters met the threshold to be billed as inpatient stays, revealing substantial revenue leakage and statusing inaccuracies.

Augmenting Clinical Judgment with Physician Advisory Integration

Phare UM functions as a decision-support layer designed to support hospital UM nurses and physician advisors rather than replace human determination:

  • Longitudinal Context Preservation: Unifies case context across clinical reviews, peer-to-peer discussions, secondary reviews, and downstream appeals so reviewers have an auditable evidentiary trail without toggling across disconnected systems.
  • Physician Advisory Workflows: Integrates directly into R1’s Physician Advisory Services (PAS) operations, surfacing condensed clinical summaries and evidence citations to accelerate level-of-care determinations and peer-to-peer defense.

The launch marks a key step in R1’s strategy to build a unified mid-revenue cycle platform. By linking Phare UM with prior authorizations, clinical documentation integrity (CDI), and autonomous inpatient coding, R1 aims to eliminate the administrative handoffs between clinical documentation and claim submission—intercepting denials before initial billing.
