Kouper is tackling the critical and often vulnerable period following hospital discharge. Poorly managed care transitions can lead to detrimental patient outcomes and spiraling healthcare costs, contributing to an estimated over $53B in annual avoidable readmission expenses in the U.S. Kouper’s AI-powered infrastructure is designed to improve patient engagement, automate scheduling, and capture vital data as patients move from emergency rooms, inpatient facilities, and urgent care settings back to their homes or other care environments.
