Fidelis Care Taps Cityblock to Deliver Comprehensive Care to New Yorkers with Complex Needs

– Fidelis Care, a leading health plan serving over 2.5 million members in New York, announced a partnership with Cityblock, a value-based healthcare provider.

– The strategic collaboration aims to provide comprehensive, community-based care to thousands of eligible Fidelis Care members in the New York City and Long Island areas who face complex medical and behavioral health conditions.

Cityblock’s unique care model connects members to a dedicated team of healthcare professionals, including primary care providers, behavioral health specialists, community health partners, nurse care managers, and more. This multidisciplinary approach ensures holistic care that addresses both physical and mental health needs.

Fidelis Care members enrolled in the Medicaid Managed Care and HealthierLife – Health and Recovery Plan (HARP) plans now have access to Cityblock’s integrated services. This includes:

  • Primary care: Regular check-ups, preventive screenings, and chronic disease management.
  • Behavioral health: Individual therapy, group counseling, and substance use treatment.
  • Social care: Assistance with housing, food security, and other social determinants of health.

Flexibility and Convenience

Members have the option to receive care virtually, in their homes, or at local Cityblock clinics, ensuring accessibility and convenience. This flexibility caters to individual preferences and specific needs.

Addressing Disproportionate Risks

Cityblock specifically focuses on serving individuals with complex needs, a population often at higher risk for poor health outcomes. Their 24/7 access and multidisciplinary care team approach aims to:

  • Reduce unnecessary emergency department visits and inpatient admissions.
