Yes Health and Solera Health Partner to Provide On-Demand Mobile Diabetes Prevention Tools

Yes Health, an all-mobile program to prevent type 2 diabetes has teamed up with Solera Health, an integrator of chronic disease prevention and management solutions to provide on-demand mobile diabetes prevention tools and resources to millions of eligible health plan members. The strategic partnership will enable qualified, at-risk patients with access to Yes Health’s Centers for Disease Control and Prevention (CDC)-recognized Diabetes Prevention Program (DPP), as a covered medical benefit.

Under the terms of the agreement, Yes Health will join Solera’s integrated network of partners who are arming individuals with the tools and resources necessary to prevent the onset of type 2 diabetes.

In its role, Solera makes chronic disease prevention programs affordable, accessible and efficient by connecting at-risk individuals with a diverse range of lifestyle modification programs as a covered medical benefit. Solera’s proprietary analytics model proactively identifies the “best fit” program provider based on individual’s unique needs and preferences. This approach has demonstrated a significant impact on improved patient outcomes at a fraction of the cost of traditional medical care.
 
As a result of the Solera and Yes Health partnership, qualified patients will now have greater access to Yes Health’s high-engagement, lifestyle-change platform that has proven to be low-cost, easy to follow and successful in preventing the onset of type 2 diabetes for all audiences.
 
“We are thrilled to partner with Solera and give millions of at-risk patients access to real-time information and resources that can help them develop healthy habits that last a lifetime and prevent diabetes,” said Alex Petrov, Founder and CEO of Yes Health in a statement. “Delivering ‘in-the-moment’ live coaching on a participant’s smartphone when they share a meal photo or during a fitness or well-being activity, is like having a nutritionist and fitness trainer, right by their side, when it counts.”
 
With half the U.S. adult population suffering from prediabetes or diabetes, Yes Health is designed to empower people everywhere to prevent type 2 diabetes and start living healthier, happier lives. Annual benefits include:
 
– “In the moment” one-on-one coaching – A personal nutritionist, fitness trainer and well-being expert is accessible anytime, on member’s smartphones.
– Helpful, real-time nutrition and meal feedback – Members snap and share meal photos get instant tips on how to make them healthier, and even swap recipes for future meals.
– Exercise motivation – Like having a trainer right by their side, cheering them on, Yes Health coaches offer fun ideas and encouragement to keep members moving.
– Stress reduction – Coaches offer simple ways to help lower stress and make more time for the things people love, all through the member’s mobile device.
– Instant progress-tracking – The easy-to-use app helps members meet their personal milestones and connects to a digital scale and starting in January 2017 to a Fitbit activity tracker, delivered to Yes Health participants at no cost to them as part of the program. 

 
Over the past year, thousands of people with prediabetes have used Yes Health to successfully reduce their weight and increase their physical activity. On average, weight loss has ranged between 5% and 9%, with activity levels that have met or exceeded the weekly DPP target of 150 minutes.
