Allscripts Integrates with TeleTracking to Improve Care Coordination

Ambulatory EHR provider, Allscripts announced it will integrate with TeleTracking Technologies including a reseller agreement to make it easier for clients to purchase both solutions. As the healthcare industry transitions from fee-for-service to fee-for-value, and new reimbursement models incentivize providers to coordinate care, organizations seek solutions to optimize both acute and ambulatory resources and better manage the discharge process.

The integration between Allscripts and TeleTracking will enable healthcare systems to optimize clinical resource utilization, deliver tools that close gaps in care, reduce avoidable costs (such as hospital readmissions), and bring enhanced efficiency to care transition and referral workflows.

TeleTracking Improving Patient Flow 

By bringing real-time enterprise visibility to patient care demand and optimizing the discharge planning process, TeleTracking improves patient flow, which creates capacity and allows for an improved patient intake process. In addition, by optimizing workflow from the time patients arrive to the time they leave, healthcare organizations can provide better care, decrease overall length of stay and serve more patients. By tracking and trending the use of assets along that journey, TeleTracking brings healthcare leaders a complete picture of how to improve resource utilization and eliminate unnecessary costs.

Allscripts Streamlining Care Management

Allscripts Care Management simplifies, streamlines and optimizes care management processes within the hospital and across multiple settings. It is used by thousands of care managers at more than 950 hospitals nationwide to improve patient care, generate higher revenues, and provide maximum connectivity.
