Physician First ACO Selects eClinicalWorks for Population Health Management

eClinicalWorks, a leader in ambulatory healthcare IT solutions, announced that Central Florida’s Physician First ACO has selected eClinicalWorks Care Coordination Medical Record® (CCMR) for advancing its physician-led Accountable Care Organization (ACO) objectives across 22 practices, including coordinating care across the continuum, evaluating population health and quality improvement activities. Physician First was named an ACO earlier this year by the Centers for Medicare & Medicaid Services (CMS).

“Physician First ACO brings quality, coordinated care to its patients,” said Dr. Bishnu Verma, CEO of Physician First ACO. “As a physician-led ACO connecting 22 independent practices, we need a system that can help us increase our savings while also improving the quality of care. We selected eClinicalWorks because it is a true partner, sharing our goals. The eClinicalWorks population health management platform will give us more insight into our patient populations and the ability to streamline care coordination across providers.”

The ACO care delivery model coordinates care between patients and members of the healthcare team with the goal of helping strengthen the physician-patient relationship, leading to better care and reduced avoidable costs over time. Physician First ACO participates in the Medicare Shared Savings Program, which has been created by CMS for facilitating coordination between providers to improve the quality of care for Medicare Fee-For-Service (FFS) beneficiaries, improve patient satisfaction and reduce unnecessary costs. eClinicalWorks CCMR is a Web-based, population health management platform for supporting ACOs along with other shared-savings models and will aid Physician First ACO in meeting its quality measures and performance metrics.

eClinicalWorks CCMR is designed to provide visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payor claim feeds, including CMS, to provide cost and utilization for services delivered to patients.

Included in CCMR, eClinicalWorks Population Health Management Analytics uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, for including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
