TriZetto and Wellcentive partnership offers advanced solutions for collaborative care.

TriZetto and Wellcentive, Inc. today announced a partnership to offer solutions that enable payers and providers to more effectively collaborate through accountable care initiatives to drive improved clinical and financial outcomes.
TriZetto’s newly launched collaborative care solution, powered by Wellcentive, will provide actionable intelligence to execute highly-impactful accountable care programs for improving both clinical and financial outcomes.

“TriZetto helps healthcare organizations automate key administrative processes as well as improve the cost and quality of care,” says Jeffrey Rose, MD, chief medical information officer for TriZetto. “The future of healthcare lies in increased collaboration that enhances the delivery of care and enables focus on value over volume. We’re committed to being a powerful ally for our clients by providing solutions that improve outcomes, and this partnership with Wellcentive strongly supports that goal.”

“Wellcentive has built its success by delivering comprehensive population management solutions for providers, and this partnership with TriZetto creates a powerful solution to support the collaboration of payers and providers in the delivery of higher quality care” says Tom Zajac, president and CEO, Wellcentive. “Combining our experience in aggregation, analytics, and action, with TriZetto’s integrated solutions for benefits administration and care coordination, will provide the solution for any healthcare organization looking to improve clinical and financial outcomes.”

By aggregating and normalizing data from multiple clinical and administrative systems, the new solution facilitates coordinated, provider-led chronic condition management and preventive care, while offering views of health status at the population, practice, and patient level.

The partnership augments TriZetto’s already extensive portfolio of capabilities that foster payer-provider collaboration through more efficient provider network management, advanced payment bundling capabilities, and real-time point-of-service patient payment calculation. “Our goal has always been to provide our clients with solutions that meet their unique requirements to help enhance the experience they deliver,” Dr. Rose points out. “Through our unique ability to connect payers and providers and by leveraging advanced solutions via partnerships with industry-leading organizations like Wellcentive, we are delivering new means of improving efficiency and effectiveness and enabling higher-quality, more accountable care.”
