Practice Fusion Partners with Practice iQ to Help Physician Groups Perform as ACOs

Practice Fusion, has teamed up with Practice iQ to provide an affordable easy way for a group of primary care physicians to participate in value-based reimbursement and benefit from health reform. 

As medical organizations transition to value based care models, like Accountable Care Organizations (ACOs) and Patient-Centered Medical Homes (PCMHs), this agreement will create an advanced technology solution that can help doctors improve their effectiveness in managing both the cost and quality of patient care.

ACOs and PCMHs can significantly improve both the quality and cost effectiveness of healthcare; however, they also require doctors to make changes in their workflow and employ technology that provides more complete and actionable information regarding their patient populations. Combining the Practice Fusion EHR and the health care technology from Practice iQ helps to ensure doctors receive the clinical information they need at the time they need it most within their current workflow.

Practice iQ, a Healthagen business within Aetna, offers a suite of payer-neutral care management, integrated health care technology and business services to help independent physician groups operate and succeed with a range of value-based reimbursement models.  Included among the resources these groups need to transition from volume to value are population health management tools that allow doctors to monitor the health of their whole patient population and improve care quality.

With Practice Fusion’s user-friendly, free web-based EHR, physicians migrating to a value-based care model have a simple, digital platform with built-in connectivity to the tools powering their ACO or PCMH.
