Vree Health infographic illustrates a comprehensive approach to transitional care that’s built on four solid pillars of success to reduce readmissions.

The cost of readmission’s is an increasing problem in the healthcare industry, costing our country $26 billion a year.  Reducing 30 day hospital readmissions has long been a priority to avoid burdening our healthcare system with unnecessary costs. The CMS penalties for excess readmissions implemented last October has only increased the focus on a solution.

While a number of interventions have been tried–ranging from promising new telehealth and remote monitoring technologies to good old-fashioned personal support, such as home visits–no clear solution has emerged to the $17 billion problem. According to data published by CMS in April 2013, there were 70,000 fewer Medicare readmisions in 2012. The results are promising, but we still have a long way to go.
