Turning Hospitals into Regional Data Hubs

The core challenge of value-based care is that patient data is incredibly fragmented. A community hospital may know what happened during a patient’s emergency room visit, but they are often blind to what happened at the behavioral health clinic, the post-acute care facility, or the local community services center.

The Azara DRVS platform solves this by acting as an aggregation layer. It pulls data across the entire care continuum—fusing clinical EHR data, practice management systems, health information exchanges, and Social Drivers of Health (SDOH)—into a single, actionable dashboard.

For hospitals utilizing MEDITECH Expanse, tt allows these rural and community hospitals to essentially serve as the “data hubs” for their regional care networks.
