Form Health Selected for Novo Nordisk’s NovoCare® Platform to Expand Obesity Care

What You Should Know: 

– Form Health, a provider of science-based obesity and cardiometabolic care, has announced its selection as a healthcare provider option on Novo Nordisk’s NovoCare® platform. 

– The NovoCare® platform connects patients with trusted healthcare providers for chronic care treatment. Form Health’s inclusion will expand access to high-quality, evidence-based care delivered by trained Obesity Medicine Clinicians and Registered Dietitians.

Form Health’s Differentiated Clinical Model

Form Health is one of a select group of telehealth providers on the NovoCare® platform. The providers were chosen for their commitment to following clinical guidelines and delivering dedicated, evidence-based support.

The Form Health clinical model consists of one-on-one, personalized care delivered by a team led by an ABOM-certified physician. The care teams also include Advanced Practice Providers and Registered Dietitians with specialized training in obesity treatment.
