– Vivalink, a provider of digital healthcare solutions announced its collaboration with the Connected Health Collaborative Community (CHCC) program to create a national framework for hospitals and health systems to develop, implement, and scale Hospital-at-Home (HaH) programs.
– The comprehensive blueprint, titled “Advancing a Sustainable Hospital-at-Home Ecosystem at Scale,” was developed alongside leading health systems, payors, and technology partners. The CHCC program is co-hosted by the Digital Medicine Society (DiMe) and the Consumer Technology Association (CTA), with sponsor UMass Chan Medical School’s Program in Digital Medicine.
Urgent Need for Sustainable Hospital-at-Home
The Acute Hospital Care at Home (AHCAH) program, which allows in-home care to be reimbursed at the same rate as in-person care, is set to expire at the end of September 2025. This leaves hospitals without a clear reimbursement path for sustaining this care model. If Congress does not extend the waiver, health systems risk losing the financial support needed to fund these programs, potentially leading to a pause or scale-back of HaH initiatives just as the model gains traction in the U.S.. HaH programs have a proven track record, having been established in other countries for decades, and have led to improved patient outcomes and lower costs in the U.S. since the waiver’s introduction.
