–  Emory Healthcare, Georgia’s most comprehensive academic health system, is collaborating with Aidin, a leading care transitions platform, to streamline the post-hospitalization care journey across its network.

–  This systemwide rollout will optimize patient placement into post-acute care based on location, availability, and insurance coverage—ultimately reducing hospital stays, easing administrative burdens, and improving outcomes. 

Emory Healthcare Partners with Aidin to Streamline Post-Acute Care and Accelerate Patient Discharges

Emory Healthcare has adopted Aidin’s digital platform across its 10 hospitals, Emory Clinic, and affiliated post-acute providers to improve care coordination, accelerate discharges, and enhance patient experiences. This partnership positions Emory among a growing cohort of major academic and integrated health systems nationwide using Aidin’s infrastructure to advance value-based, patient-centered care.

By implementing Aidin’s platform, Emory aims to achieve the following strategic outcomes:

·       Improved patient decision-making and care outcomes: Aidin equips patients, families, and caregivers with tools to make more informed choices, helping them select the most appropriate post-acute care providers.

·       Faster hospital discharges: The platform facilitates collaboration with post-acute care providers and payors through a curated, data-informed approach, reducing hospital length of stay.

·       Reduced administrative workload: Aidin streamlines communication and automates workflows, allowing hospital staff to spend less time on manual coordination and more on direct patient care.
