What You Should Know:

–   Lumeris, a leader in healthcare technology and services, today announced the integration of Wolters Kluwer Health UpToDate®, a market-leading clinical decision support solution, into Tom™, the industry’s most advanced AI-powered Primary Care as a Service™ solution.

– The collaboration aims to support overburdened physicians with real-time, evidence-based recommendations delivered directly within their clinical workflows at point-of-care, enhancing decision-making and improving patient care.

Lumeris and UpToDate Partner to Strengthen AI-Driven, Scalable Primary Care Delivery

Founded in 2012, Lumeris is a leader in value-based care transformation, partnering with health systems and physician practices nationwide to implement primary care-led models that improve quality and reduce costs. The company’s AI-driven platform, Tom™, powers this transformation by enabling proactive, data-informed clinical decisions and operational efficiencies across Medicare Advantage, Traditional Medicare, Medicaid, and Commercial populations. In 2024, Lumeris generated $3 billion in revenue and employed 1,200 engineers, healthcare specialists, and clinicians across offices in St. Louis and Boston. Its Medicare Advantage plan, Essence Healthcare, has consistently earned 4.5 to 5.0 star ratings from the Centers for Medicare and Medicaid Services.

The U.S. primary care system is increasingly overburdened, with over 100 million Americans lacking access to a primary care provider and an annual supply-demand mismatch exceeding 2 billion hours. According to Dr. David Carmouche, Chief Clinical Transformation Officer at Lumeris, “This gap cannot be closed with human resources alone. Integrating the power of Tom with UpToDate will significantly narrow the care access divide.”

Tom™, Lumeris’ AI-powered Primary Care as a Service team member, leverages deep data insights and trained clinical models to identify and autonomously execute best-next actions. These range from scheduling preventive screenings and monitoring medication adherence to conducting patient outreach and escalating clinical issues. With the recent integration of UpToDate, Tom™ now delivers evidence-based recommendations at the point of care—enhancing clinical decision-making, reducing provider burden, and improving patient outcomes.

This collaboration supports Lumeris’ mission to deliver accessible, high-quality primary care through scalable AI solutions. Key benefits of the Tom™ and UpToDate integration include:

  • Comprehensive patient insights, with AI synthesizing EHR and claims data into a 360-degree view of each patient’s needs.
  • Personalized decision support, aligning Tom™’s analytics with UpToDate’s clinical guidance for actionable recommendations at the point of care.
  • Seamless workflow integration, enabling physicians to receive insights directly within clinical workflows through Visit Assistant, without added complexity.
  • Improved outcomes at scale, reinforcing Lumeris’ broader objective to expand guideline-based, tech-enabled primary care nationwide.

Lumeris continues to expand its ecosystem of collaborators to promote the responsible use of AI in healthcare and further its commitment to scalable, patient-centered solutions.
