Citizens Memorial Hospital Integrates Unite Us with MEDITECH Expanse EHR

What You Should Know: 

– Citizens Memorial Hospital (CMH) in Bolivar, Missouri, has announced a collaboration with Unite Us, a technology partner for social care transformation, to integrate its MEDITECH Expanse EHR with Unite Us’ closed-loop referral system. 

– Citizens Memorial Hospital is a comprehensive rural healthcare system serving eight counties in southwest Missouri. The integration aims to identify and address the social care needs of CMH’s patient population, ultimately improving community health.

Seamless Integration Streamlines Social Care Referrals 

By seamlessly integrating Unite Us into MEDITECH Expanse, CMH’s clinical providers can now access patients’ social care history, build and scale a coordinated care network, track outcomes, identify service gaps, and monitor at-risk populations—all within their familiar EHR workflow.

The Unite Us platform enables Citizens Memorial Hospital to securely connect individuals in need with community-based resources through the Unite Missouri network. This platform acts as a central hub where health and social services providers can securely access and refer patients to necessary services, while also monitoring their progress and measuring outcomes.

CMH holds the distinction of being the first MEDITECH customer to implement Unite Us seamlessly within Expanse.
