Aledade Adds 500 New Primary Care Practices to Value-Based Care Network

What You Should Know:

– Aledade, the nation’s largest network of independent primary care providers, announced the addition of over 500 new primary care practices, clinics, and community health centers to its network.

– The expansion brings the total number of supported organizations to more than 2,400, serving nearly 3 million patients through value-based care programs across the United States.

Fueling Nationwide Adoption of Value-Based Care Models

Aledade partners with primary care organizations of all sizes, including over 300 community health centers, many of which are located in underserved areas. Aledade’s growth is driving significant adoption of value-based care models, particularly in the Medicare Shared Savings Program (MSSP). Notably, 83% of Aledade’s new MSSP participants are joining the program for the first time, highlighting the company’s role in expanding access to this innovative care model.

Aledade now supports over 2,400 primary care organizations in 46 states and the District of Columbia. The company serves nearly 3 million patients through various value-based care programs. Aledade is responsible for nearly one in five new practices participating in the MSSP.

“Since day one, Aledade has led the charge in enabling independent primary care doctors nationwide to adopt and thrive in value-based care, enhancing patient outcomes while ensuring practice sustainability,” said Farzad Mostashari, M.D., co-founder and CEO of Aledade. “With every new practice that joins the Aledade network, we gain a greater ability to improve patient outcomes, support practice growth and move health care forward nationwide.”
